Abstract
This open access paper, the laboratory system and the work it reports relates to a standardised procedure for the reporting of incidents in labs. Such a system could have tangible benefits for institutions, labs, research teams and for the integrity of science. This is a recommended read for senior staff in labs and staff in research governance teams.
